New Patient Registration

To register at Parklands Surgery, you will need to complete and sign an NHS family doctor registration form.  This will allow us to register you with Parklands and to obtain your medical records from your previous practice.  You will also be asked to show some form of photographic identification, such as a passport, and proof of address, such as a recent utility bill, to show that you live within the surgery boundary.

Your Medical Records

When you register, you will also be asked to complete a patient questionnaire for yourself and each member of your family.  This information will provide us with details of your medical history and relevant lifestyle which will help us until your medical records arrive.

Assessment Medical

All patients joining the Practice are encouraged to have a medical assessment with one of the Healthcare Assistants.  During the medical, we will ask you for a urine specimen for testing and will record your blood pressure, height and weight.  We will ask you some questions about your health and record any chronic illnesses you may have.  Please bring with you any details of current medication and allergies.  This assessment is very useful to us.

Change of Address/Telephone Number

Please let us know as soon as possible if you change your address or telephone number.

Visitors from Overseas

Visitors from outside the United Kingdom, who are temporarily resident in the Practice area, may be requested to pay for any consultation and subsequent treatment unless a reciprocal arrangement exists between their country and the UK.
